Before Monday

Print 5 forms. Single-sided, black ink.SS-5 for Ella, Lia, you. DS-11 for Ella, Lia. Sign nothing yet.

Two passport photos. One per girl.2"×2", white background, taken within 6 months. Any pharmacy does it. Infant photos are fine.

Photocopy the certificates and both parents' passports.One set each.

Stop 2 — Passports

Monday 7:30–4:00. Appointment only. Both parents and both girls. Security screening, then the 1st-floor information window. 10 minutes by cab from Stop 1.

Pack it, per girl: DS-11, certificate plus copy, Israeli birth certificate, photo.DS-11 stays unsigned until the agent says sign.

Execute Ella's DS-11.

Execute Lia's DS-11.

Pay $195 per girl.$100 application, $35 execution, $60 expedite. The certificate stays with them — it comes back with the passport.

Get the pickup date in writing before you leave.

After

Cards land in Teaneck in about two weeks. Get the numbers.

Add the SSNs to Form 4547. Sign. File.Ella's $1,000 box is already checked. Lia gets the account — born 2024, no pilot money. That's the rule, not us.
